Student Withdrawal to Zero Credits Form

Current Students Name Change Form
Students needing to legally change their name must complete the Legal Name Change Request 校园连接表格. On your home page in 校园连接 click on the eForms tile. In the margin on the left hand side, click “Legal Name Change Request" and then complete the name change form.

Former Student Name Change Form
This form is required to make any changes to your name. Proper adjoining documentation is required. (无需登入).

Request to view specific academic records. 记录 requested may be viewed in person, 邮件ed or faxed. 
 
Student Advisor/Program Change Form
This form is used to change a student's advisor as well as program of student.

If you transfer to an institution before graduating 从365ASIA亚洲版, it may be possible to combine the completed credits 从365ASIA亚洲版 with those from your new institution to earn your associate degree through reverse transfer.
 
An associate degree is a milestone during the progression to a bachelor's degree. Students with an associate degree may have higher lifetime earnings, 平均, than students with some college but no credential (Carnevale, 玫瑰, & 却,2011).

Incoming transfer students want to know how previous college-level courses fit with 365ASIA亚洲版's 毕业 requirements. College-level coursework from regionally accredited US institutions (or equivalent international institutions) is eligible for acceptance in transfer. Remedial coursework credit is not accepted for transfer, 然而, it may fulfill prerequisite requirements. Important Notice Regarding Transcripts Between North Dakota Public Colleges and Universities as of August 13, 2018: It is not necessary to request 成绩单 through this site If you are requesting 成绩单 to be sent for the purpose of admission to any of the North Dakota University System (NDUS) institutions for the purpose of being admitted to a NDUS college or university. 的 NDUS institution to which you have applied will retrieve your academic transcript at no charge to you. Administrative Hardship Withdraw

Students Reported as Non-Attending
Students who are reported for courses as non-attending based on the important dates rubric, will be charged 25% of the tuition and fees for that course and receive a W on their transcript.
